Declaration of Residency/Caregiver Declaration

Families who live in a residence other than their home residence (i.e. living with another member of the family or living with a friend) must come to Student Support Services and complete a Declaration of Residency form. In order to complete a Declaration of Residency, the owner/leasee must be present at the time of the appointment.  In addition, the owner/leasee must show two proof of residency (current rental agreement/mortgage/property tax AND one additional proof of residency such as a recent utility bill, pay stub, voter registration, or correspondence from a government agency).  The parent/guardian and owner/leasee must also present photo identification (e.g. government issued identification).

Caregiver Declaration - Students living with someone other than their parents, that don't have legal paperwork showing guardianship, must complete a Caregiver Declaration.
