Medication at School

Students who need to take medication at school, must have the required district forms with doctor's signature on file. Medication forms must be renewed each year. Prescription medication, such as antibiotics or inhalers, and over-the-counter drugs, such as aspirin or Tylenol, may not be carried or administered at school without the proper forms.

This form is required for school personnel to assist with administration of medication at school or if student will self-administer or carry medication. This form applies to over-the-counter and prescription medications. Examples include but are not limited to Albuterol inhaler, Tylenol, ADHD medication. FORM E is also required if student is authorized to self-administer or carry medication.

FORM E/SELF ADMINISTRATION OF MEDICATION CONTRACT (pdf)

This form is required in addition to FORM D if student will self-administer or carry medication. FORM E is signed by the student and parent.


On the last day of school, please remember to pick up any medication that was stored at school. Medication left behind will be disposed of according to district policy.
