2024 - 2025 California School Health Services Survey
Welcome Message and Instructions - IMPORTANT, PLEASE READ THOROUGHLY
Thank you for your support in providing data on the health services offered in your district. This data is crucial for informing the development of school health programs, updating stakeholders involved in student issues, and outlining the range of health services provided by school nurses statewide.
Preparation worksheets are provided to facilitate data entry. Before starting the online report survey, please download the worksheets and mark the appropriate responses. DO NOT click into the survey until you are ready to submit. The data worksheet matches the survey exactly so there is no need to click into the survey to look at anything!
This is a district or single school-level survey. While district-level is preferred, if you are a nurse in a school or several schools, please submit your data even if your district does not.
For district-level submissions, have one person (i.e., the official or unofficial lead, a nurse administrator, or someone interested) complete it for all schools in the district (County Offices of Education are considered districts). Review and share the worksheets with all school nurses in your district to ensure consistent data collection for the designated data entry person to compile.
Please complete the survey to the best of your ability. Please avoid estimates; it is important to have numbers that are as accurate as possible. If you encounter data not collected in your district or that you do not have access to, leave it blank or enter a '0.' Please plan to collect or seek access to this data in the future so that you can enter numbers in the future.
Survey completion should take ~90 minutes, depending on your current data collection practices (this is time required to enter numbers into the survey, the time to collect and compile the data will be longer). The first time submitting will take additional effort and time but will eventually become less cumbersome. Participation is voluntary, responses are anonymous, and individual answers will not be shared (only aggregate results will be reported).
