Working Papers
New Jersey law requires all workers under the age of 18 to obtain working papers for employment.
This process is now all online---more information can be found on this handout.

Transcripts
PRESENT STUDENTS: During the school year, present students should email your guidance counselor to have an official transcript sent to a college/university directly. During the summer, email Mr. Galatola at rgalatola@frhsd.com.
Please note that official transcripts can only be sent to colleges, universities, or employers. Non-official transcripts can be printed from Genesis directly (Grading tab).
FORMER STUDENTS: To request a transcript for former students, complete and submit the Graduate Transcript Request Form to the HHS Guidance Department Office via fax 732-751-1571 or scan and email to ammcdonald@frhsd.com.

Address Changes
Print out and fill out this Change Address form. Fax or email scanned form back along with providing 3 scanned documents as proof of the new residence (i.e. utility bills, signed leased, closing papers, etc.).

In the case of joint custody, the Freehold Regional High School District will only provide transportation to one address.
Registration for New Students
Parents are asked to contact our school guidance office by emailing Ms. Lykes at jaclynlykes@frhsd.com to schedule a registration appointment. Schools cannot be entered without an appointment because of security.
Ms. Lykes will email you the necessary forms to be filled out. The following documents should be gathered before registration can be completed:
Health Records/Immunization Card
An original birth certificate with a raised seal from the town where your child was born (not the hospital certificate)
Three proofs of residency (deed, mortgage, lease, tax bill, certificate of occupancy, current utility bills, etc.) – a driver’s license will not be accepted as a proof of residency
The most recent report card (or unofficial transcript) showing course and grades and any standardized test results.
Current 504 Plan or IEP, if applicable
If the student does not reside with both parents, a copy of custody papers or notarized guardianship papers must be provided.
If the student resides with anyone other than his/her parents or court-ordered legal guardians, guardian affidavits must be submitted.
If the student and parent are temporarily residing with relatives, etc. who reside in the district, the parent must provide a notarized statement signed by the parent/guardian and owner or lease with whom they are residing. The parent must then provide proof of residence within 30 days.
