IMPORTANT DATES AND INFORMATION FOR 2022-2023 SCHOOL YEAR

Required Immunizations for 6th Grade:

The NJDOE and the American Academy of Pediatrics have not changed immunization schedules and requirements due to the COVID-19 pandemic. Please contact your healthcare provider to schedule your child's immunizations. Per New Jersey law, students have 14 days from their 11th birthday to obtain the Tdap and Meningococcal vaccines. Students that turn 11 years old before August 26, 2022, must submit proof of immunization in order to begin school in September.

Beginning 7/5/22, completed sports physicals and immunizations can be dropped off at The Board Office, 284 West Main Street. Tuesday-Thursday 8:30am-4:00 pm

Forms may also be mailed to Barkalow to the Attention of your Middle School Nurse. PLEASE DO NOT SUBMIT SPORTS PHYSICALS TO YOUR ELEMENTARY NURSE.

If you are unable to submit your forms by the deadline, please contact your middle school nurse.

MEDICATION: All medications administered by the school nurse require a physician's order and parental consent form to be submitted to the Health Office. These must be resubmitted each school year. Please print the applicable health forms that are attached and complete with your physician. ***Allergy Action Plans are required for life-threatening allergies, ***Asthma Action Plans are required for inhalers (including self-administered). EpiPens with a dose of antihistamine/Benadryl and inhalers are the only medications that students may carry and self-administer, per New Jersey law. Please complete a Medication Order form for other necessary medications that will be stored in the Health Office and administered by the Nurse. Please check your emails during the summer for dates and times for drop off in late August.

MEDICAL NOTES / EXCUSES: Physician's notes are required to excuse a student from Physical Education class for more than one day. A parent note is required for a gym excuse for a single day. Student should bring gym notes to the Health Office when they arrive at school. Notes should indicate a diagnosis and must include a length of time that the student is restricted from activity. Students will not be permitted to return to Physical Education/sports until written clearance is obtained from a physician. If your child is excused from gym class, he or she is also excused from sports.

SPARE CLOTHES: I encourage ALL students to keep a spare set of clothes in a string bag in their lockers "just in case."

The Scholastic Student-Athlete Safety Act (SS-ASA) (N.J.S.A. 18A:40-41.7) mandates that sports physicals may ONLY be completed by a licensed physician, advanced practice nurse (APN) or physician assistant (PA) who has completed the Student-Athlete Cardiac Assessment professional development module.

The licensed physician, APN or PA, must sign the certification statement on the Pre-participation Physical form attesting to the completion of the professional development module.

Physical forms will not be accepted without the healthcare provider’s signature and stamp in the certification statement section.

The student will not be cleared to participate in their sport unless your healthcare provider completes the Student-Athlete Cardiac Assessment professional development module AND signs the certification statement on Page 4.

It is recommended that you verify that your medical provider has completed this module before scheduling your sports physical appointment.

THE DEADLINE TO TURN IN FORMS FOR FALL SPORTS IS August 26, 2022.

THE FALL SPORTS SEASON STARTS ON SEPTEMBER 13, 2022

Exams conducted on or after 9/14/21 are acceptable for the FALL 2022 season.

The Annual Athletic Pre-Participation Physical Examination Forms are required by the New Jersey Department of Education. These forms are required for any middle or high school student in New Jersey who wants to participate in an interscholastic or intramural sports program. Students must have their exam approved by the school doctor before they can participate in any practice or tryouts. Our school doctor is scheduled in advance to review all the sports physicals for the entire district. Forms that are submitted late or incomplete cannot be guaranteed to be approved by the school physician before the season starts. Students who have not received clearance by the school doctor may not participate in any practice or tryout. Please adhere to all deadlines and guidelines to ensure that your child is able to participate.

  • Must be completed in full by the PARENT/GUARDIAN

  • Must explain all “yes” answers; include dates, length of symptoms and treatment. You may attach a second piece of paper if more room is needed

  • The parent/guardian and student must sign and date

  • The date on the History form must be the same date as the physical exam *This form must be reviewed by the health care provider at the time of examination

  • Must be completed in full by the HEALTH CARE PROVIDER

  • Both forms must be signed, stamped and dated by the health care provider

  • All “indicators” must be filled in by the examiner; this includes height, weight, blood pressure, pulse and vision.

  • Health care provider signature in the Cardiac Assessment Professional Development section.

  • INCOMPLETE FORMS WILL BE RETURNED FOR COMPLETION BY YOUR HEALTH CARE PROVIDER PER NEW JERSEY ADMINISTRATIVE CODE.

  • Exams conducted on or after 9/15/20 are acceptable for the FALL 2021 season.

  • This form must be signed on the Genesis Parent Portal each sports season.

  • Dated no more than 90 days from the first practice/tryout

  • Must be completed in full by the PARENT/GUARDIAN

  • Complete 1 form PER SEASON not sport.
