You will have to complete Section 1 of the form below, which includes your name, SSN, business address and email. You can leave the Humana Agent Number blank, unless you are already contracted with them and you are doing a transfer of a contract.
You do not have to enter anything on Section 2.
For Section 3 you will have to apply a wet signature. Note that an electronic signature will NOT be accepted.
Once you have filled and signed the form, email it back to contracting@freedompolicy.com.

Once we submit your ABTF to the carrier you will soon (matter of couple of days) will receive a Humana Contract Invitation email like the one below.

Once Humana receives your application, it will take 5-7 business days to notify you about the status of your contract. If this is approved you will receive a Welcome to Humana email in one of the formats below.
