If you are NEW to Franklin County Schools, you will need to go to the school in your area to register your student on the kiosk in the office. Your student’s school will assist with any questions you may have.
To complete OLR you will need your child’s:
Health history
Medications
Allergies
Physicians name and number
Emergency contacts– full legal name, date of birth, and phone number for each contact
Please make sure you have the following information to turn into your child’s school:
Child’s Birth Certificate
Proof of Residence (mortgage, statement or current lease agreement in your name or a secondary document such as a current utility bill)
Proof of your child’s recent preventative health care exam
Up-to-date Kentucky immunization certificate
Proof of an eye exam
Proof of a dental exam
Court documents pertaining to your child, if applicable
Social Security Card
Health Insurance Card
***Preschool students must be screened and meet criteria by the district office prior to registration.***
