All students are required to comply with state mandated physical examination requirements to attend school. All new students are required to complete the FPSD New Student Physical Examination Form prior to entrance. For grade-specific physical examination recommendations, please refer to the below link. Please share a copy of your child's most recent immunization record. Please see here for more information regarding New Jersey Immunization Requirements.

This form is used for administration of most medications, including ADHD medications, antibiotics, pain relievers, etc. For epinephrine, antihistamines and steroid orders written for food, medication and/or bee sting allergies, please choose the form entitled: "Allergy Action Plan".
This form allows students to self-administer inhalers, epinephrine, and insulin as per NJ state regulations. If you feel your child is capable of self-medicating in an emergency situation, this form must be completed by you and your child's physician.
If your child has a life-threatening FOOD allergy*, these forms are to be completed by both the doctor and parent/guardian.
*For NON-FOOD life-threatening allergies (e.g., insect stings, latex, etc.), contact your child's school nurse for the appropriate forms.
This form is for students who have a history of asthma. It is completed by both the doctor and parent/guardian. If your child has a food allergy that can exacerbate their asthma, please include this form as well.
If your child has diabetes, this form is completed by both the doctor and parent/guardian. Please contact your child's school nurse for additional guidance.
This form is for students who have a history of seizures/epilepsy. It is completed by both the doctor and parent/guardian. Please contact your child's school nurse for more details.
Concussions & Head Injury Information:
