Tryout Packet

You will need the following items to tryout on April 1st

• Completed application signed by a parent/guardian.
• Physical Form– Must be dated for the 2019 – 2020 school year (after February 1st, 2019) Please use attached physical and ensure all areas are filled out.
  • Make sure everything is filled out correctly and signed.
• Copy of your 2^nd qtr. report card.
• Teacher recommendations (2) you can just bring them with you to the tryout in a sealed envelope.
