Waiver Guidelines

To request a change in your student's courses for 2025-2026 you must COMPLETELY fill out the information in the form.  The waiver form is found on the high school's website that the student is attending.

A separate request for EACH course change request must be filled out; multiple course changes cannot be handled in one form.
