Compliance Forms

Compliance forms only required for NEW students starting in January.  Only need to fill out once each year.

Sign into the PARENT FOCUS PORTAL to complete the compliance forms. (https://flvs.focusschoolsoftware.com/)

Username will be the email address used when creating the parent portal account. 

These forms will need to be completed for each student enrolled at FLVS.
