Compliance forms are required for ALL students and only need to be  completed once each year.

Sign into the PARENT FOCUS PORTAL to complete the compliance forms. (https://flvs.focusschoolsoftware.com/)

Username will be the email address used when creating the parent portal account. 

These forms will need to be completed for each student enrolled at FLVS.
