Applying for a Fox Lake District Library Card
Fox Lake District Library cards are free for district residents and provide you with access to Library materials, as well as our digital resources.
The Fox Lake District Library serves most of Grant Township, Illinois.
Fox Lake, Ingleside, portions of Lakemoor, Round Lake, Spring Grove, Volo, and also small portions of Antioch and Burton Townships are within our District.
If you're not sure if we are your home library, please contact our Circulation Desk and we will be happy to check your address. Your library card can be used at any public library in Illinois.
What if I am already a Fox Lake District Library Patron?
Expired card?
New library cardholders and patrons renewing their cards this month will get one "Proud Library Cardholder" yard sign and a swag bag!
Lost card?
During Library Card Sign Up Month we waive replacement card costs.
Fines?
We’ll waive them with the Food for Fines Program. See the summary here.
Online Borrower Registration
This form is for NEW card registration ONLY.
Do not use this form to attempt a card renewal.
If your card has expired, visit the Main Circulation desk with an accepted form of identification and we will be happy to assist you.
Your temporary credentials will expire 30 days after date of submission.
Please visit the Circulation desk with your ID or any other accepted forms of identification to pick up your card for your full 5-year term and to begin borrowing physical material.
You may use these temporary credentials immediately for digital services only.
What do you need in order to get a new or replacement Fox Lake District Library card?
A parent or legal guardian must accompany anyone under 18 years of age. If you are the legal guardian, you must bring legal documentation showing proof of guardianship.
Applicants must present one proof of identity and one proof of current Fox Lake Library District address, such as:
Driver’s license or state ID
State vehicle registration
Insurance Policy or ID Card
Most recent property tax bill
Formal lease or agreement (not handwritten)
Firearm owners ID (FOID) card
Alien residency card
Printed checks or bank statement*
Voter's registration card
Utility bill*
Piece of first class mail*
*Dated in the last 30 days.
These lists are not intended to be exhaustive. Other forms of documents that meet the requirements of our Circulation Policy may be accepted.
Other Important Information
If you lose your library card, immediately contact us so that we can put a courtesy block on your card to prevent others from using it. There is a $2.00 fee for replacement of lost cards. (Replacement fee waived during Library Card Sign Up Month.)
Physical Library cards expire 5 years from the registration date.
Card privileges are revoked at account balances greater than $25.00.
All account balances must be reconciled before being replaced or renewed.

Statement of Responsibility
A Fox Lake District Public Library card holder is responsible for and accepts financial responsibility for any loss, damage or overdue fines on all materials borrowed on their card. Present your card each time you check out materials. Card holder agrees to abide by the policies of the library. Please inform the library immediately of a lost card, change of address or name change. A fee will be charged for lost cards. Library card can only be used by the person in whose name the card was issued.
