2022-2023 Physical Forms Paperwork must be turned in prior to your student starting workouts, open gyms, or practices this summer and fall for Lapel Middle School. The physicals must be dated 4/1/22 or later to count for this 22-23 school year.
The final piece of the athletic physicals is the online Synergy Athletic Participation Registration. I have attached a video to help with this process. https://www.loom.com/share/c9ddb88fa0234e37ad8f3f79f4ad5e2d.
Returning athletes must be sure to have all previous dates changed to reflect the actual date that you are registering your student for this school year while completing the online form. Athlete's Medical Information, CIO Consent, Drug Testing Policy, Concussion and Sudden Cardiac Arrest, and Athletics Physical Form need to be updated.
Be sure to get physicals returned to the office in a timely manner as athletes are not allowed to participate until those are on file. Athletes must have 10 official practices in before being eligible to participate in contests. Official practices do not begin until August 1st.
