STUDENTS must fill out the form linked below if your guest is 

- Parents/Guests cannot fill out the form 

- A non FLAHS Student

- A Sophomore 

No guests will be allowed over the age of 21 or younger than a sophomore. 

The form will be available to fill out between April 10 and May 2. Forms will NOT be accepted after May 2 and your guest will NOT be allowed to attend prom. 

Guest Agreement forms will NOT be looked at by administration until May 2. Be patient with them as they look through them. It takes time. You will be contacted when your guest form has been completed by administration.
