If your child is sick or injured and will not be able to attend school please let the school know using the attendance line.  Please provide the school with medical documentation for school days missed due to sickness or injury.

Grade 7 students are required to receive TDAP and Meningococcal vaccines along with a Physical Exam.

If your child needs any medication to be given during the school day we are happy to help. In order to administer medication to your child at school please provide us with the following. 

1. Medical order from a doctor or nurse practitioner stating the medication dose and time to be given

2. Medication in the original pharmacy labeled container brought in by an adult ( no more than 30 day supply please and check expiration date) 

3. Medication administration form ( attached above or a parent/guardian can fill one out in nurse  office)                                                                                                                                                                           

These can be emailed to the nurses,  faxed to 978-343-2106, or dropped of at Memorial Middle School
