The following must be done before beginning practice on Thursday, Aug 7th:

Sports Physical. All players must have a complete sports physical prior to practicing. The form below should be printed and turned into the Davies High School main office.
Athletic Fee. Fee can be paid on PowerSchool, in person in the Davies High School main office or a wavier can be applied for by going to the Davies High School main office and requesting a form.
Concussion Form. This is located in PowerSchool (J Forms Under Parent Account--Select your child will be participating in athletics / activities)
Co-Curricular Meeting. This is located in PowerSchool (J Forms Under Parent Account--Select your child will be participating in athletics / activities)
