The HIPPA form is required for athletic training staff to communicate with coaches about athletes' health status throughout the season.
Travel Waiver and Physical Form
Every year, an athlete must complete a physical form to practice. The physical form is only necessary if the athlete has NOT competed in a sport yet this year. Please turn in your form to the main office.Â
In order for an athlete to ride home with a parent or guardian, a travel waiver must be submitted to the head coach of the team. One form is required per season (that is, you do not need to turn in a form for every game).
An annual co-curricular meeting is required for any student participating in any sort of co-curricular activity.
