End of School Year Medication Pick up
Please remember to pick up any medication that is in the school nurse’s office belonging to your child on the last day of school. These medications include: EpiPens, Inhalers, Spacers, Motrin, Tylenol, prescription medications and any other medications or over the counter products that are kept in the nurse’s office during the school year. Please note that items not retrieved on the last day of school will be disposed of and cannot be kept for the following school year. (If your child will be enrolled in the summer daycare program held at Tansey, you must retrieve the medication and deliver it to the day care program staff).
You will need to obtain a new doctor’s order for any medication to be given during school hours for each academic year your child is enrolled in the school. Prescriber orders should be dated from August through the end of June of the following year. You will need to sign parent/guardian consent forms for EACH medication delivered to school in August. Please check expiration dates on all medications before bringing them to school. Children who have documented food/insect sting/latex allergies are required to have a twin dose pack EpiPen delivered to the school nurse on the first day of school, or they may be exempt from school until obtained.
Medications must be delivered to the nurse’s office by an adult in August. Over the counter medications must be delivered in an unopened box. Prescription medications need to be delivered in a pharmacy labeled container in a stapled bag (plan your August refill to have a separate school supply dispensed by the pharmacist).
If you have any questions please feel free to call: 508-675-8206 Fax: 508-675-4530.
Most practitioners have their own computer generated order forms which will be accepted. You may also download the MD Order and Parent Consent forms from the FRPS website.
Health Screening Follow Up
If you receive a vision, hearing, or postural screening referral notice from the school nurse; please contact your child's pediatrician to set up the appropriate evaluation by the doctor. Please have the doctor complete the referral and return the form to Nurse Whipp once it is completed so that your child's health record may be updated.
Annual Physical Exams
Please make sure your child's most recent physical exam and immunization record is on file in the health office to ensure that the health record is complete and in compliance with state law.
***PLEASE REMEMBER***
Parents of children who have medications at school will need to pick up medications on the last day of school. Any medications left at school will be disposed of with the start of summer vacation. Every school year that your child is enrolled in the FRPS, you will need to obtain a new doctor's order, signed parent consent and deliver the medication to school at the start of each new academic year.
All emergency medications such as EpiPens, Seizure and Diabetic supplies must be delivered on the first day that your child is physically present in the school building. Your child's medication, new doctor's order, and parent consent must be provided to the school nurse each new school year in order for your child to attend school.
Quarantine for asymptomatic exposed children, regardless of where the exposure occurred, is no longer required for these settings. Children who are identified as close contacts may continue to attend programming as long as they remain asymptomatic. Those who can mask should do so until Day 10. A test on Days 2 and 5 is recommended, but not required.
Children who test positive must isolate for at least 5 days. If they are asymptomatic or symptoms are resolving and they have been fever free for 24 hours, they may return to programming after Day 5, provided:
If the child is able to mask, they must do so through Day 10.
