Welcome to the Spencer Borden Health Office page!

My name is Rachel Cabral, BSN, RN, NCSN and I am the primary nurse at the Spencer Borden Health Office. Myself and a number of the Fall River District float nurses are here everyday to ensure the health and safety of our students. It is recommended that medication be administered at home. However, students who must receive medication during the school day are required to comply with MA DPH regulations. These regulations require the following forms to be kept on file in your child's health record before we begin to give any medications in school. These completed forms will need to be submitted to the PACE nurse in order to complete the registration for your child

Signed consent by the parent or legal guardian to give the medication.

Signed medication order completed and signed by your child's licensed prescriber

Please note that all medication orders must be renewed at the beginning of each academic year

Medications will only be accepted by the school nurse when brought in by a parent/guardian/designated responsible adult and the medication must be in a pharmacy-sealed bag as dispensed by the pharmacy or manufacturer-labeled and sealed container.


Please ask your pharmacy to provide separate bottles for school and for home. No more than a 30 day supply of the medication should be sent to school at one time.


Over-the-counter medications (including but not limited to Tylenol, Ibuprofen, sunscreen, cough drops) will not be given in school without the required signed medication forms.

HEALTH RECORD REQUIREMENTS

Physical Exams

A copy every student's physical exam is required:

(1) Within one year prior to entrance to school or within 30 days after school entry

(2) At intervals of either three or four years thereafter

(3) Students under 16 and over 14 years of age requesting employment certificates

(3) Prior to a student's participation in competitive athletics, on an annual basis.

Immunizations

Every elementary student must have a record of immunizations.

Exemptions

Only Medical and Religious exemptions are allowable in Massachusetts. Exemptions must be provided to the school nurse annually.

  • Medical exemptions come from the child's doctor and document a contraindication-- the reason why an individual cannot medically receive a vaccine.

  • Religious exemptions come from the parent/guardian, and state in writing that a vaccine conflicts with a sincerely held religious belief.

TB Risk Assessment

Every student is required to provide to the school nurse a completed Tuberculosis Risk Assessment, completed within one year prior to enrollment. In the case of identified risk factors, follow-up documentation is also required (i.e. negative results of a Tuberculin Test, lab testing results, or a health provider follow-up).

LEAD SCREENING

Documentation of a completed lead test is required of every child entering Kindergarten.

YEARLY HEALTH SCREENINGS

Vision Screening: All grades

Hearing Screening: Grades K-3

Height, Weight, and BMI Screening: Grades 1 and 4

Scoliosis Screening will take place for grade 5

*These Screenings are mandated by the State of MA. If you do not wish for your child to participate in annual screenings, please provide a written notice to the Health Office ASAP.
