Immunizations
All immunization must be up to date for enrollment. Ask nurse for specific requirements for each grade level.
Physical Examination
Physical Examination
A Physical Exam is required from all students dated within 1 year of school entry or within one month after school entry.
Tuberculosis Risk Assessment
Tuberculosis Risk Assessment
A TB Risk Assessment form must be completed and must be signed by your primary care provider.
Lead Screening
Lead Screening
A lead test is required of every child entering Kindergarten.
Annual Screenings
Vision Screening will take place for all grades.
Hearing Screening will take place for grades K-3.
Height, Weight, and BMI Screening will take place for grades 1 and 4.
Scoliosis Screening will take place for grade 5.
*These Screenings are mandated by the State of MA. If you do not wish your child to participate in annual screenings, please provide notice in writing to the Health Office in September.
