If so, you the parent and the doctor or nurse practitioner will need to complete a medication permit form:

If so, please provide an Epinephrine pen or Auvi-Q to keep at school, completed medication permit, and allergy action plan to the clinic.
If so, please provide an inhaler to keep at school, completed medication permit, and an asthma plan to the clinic.

Students in 9th grade are screened during the beginning of the school year. Referral letters will be sent home with those students who fail either the screening tests. If you have concerns or questions regarding your child's vision or hearing, please contact me.
If I have a concern or question about your child's condition, your child sustains a head bump, a facial injury, a dental injury, any serious injury, or has become too ill to remain at school you will receive a call. For emergencies, I will contact 911 then the parent. Please contact me anytime you have a concern or question regarding your child's health.
