WINTER sports are approaching quickly! Please review the important information below regarding athlete clearance, first practices, tryouts, and equipment distribution.
ATHLETE CLEARANCE – THROUGH FINALFORMS All athletes must be fully cleared before participating on the first day of practice or tryouts.
To be cleared through FINALFORMS, athletes must have:
Parent form completed and signed
Student form completed and signed
An up-to-date physical on file
Up-to-date insurance information
When everything is complete, all requirements in your FINALFORMS account will show green. Please take care of these requirements as soon as possible so your athlete does not miss any practice time.
🏀 Fall Basketball Preparation
Get ready for winter basketball!
This fall, we’re offering optional conditioning and open gym opportunities for students interested in preparing for the upcoming basketball season.
Sessions will begin with conditioning on the track or in the weight room, followed by open gym in the gymnasium.
Fall athletes: If you are currently participating in a fall sport, your in-season sport should remain your priority.

All students participating in athletics must have a completed online registration through Final Forms and a sports physical on file (good for 2 years from appointment date) prior to the first day of practice. Coaches are not authorized to collect paperwork or determine athletic eligibility. If you have any questions or need assistance with the online registration, or to check on a sports physical, please contact Dana Perry at dana.perry@evergreenps.org or call 360.604.6110.

Admission Prices for Home Athletic Events: Family passes are available for $150 at the Activities office. Family passes admit those who live in the same household, to all sports that require admission in the Evergreen School District. Admission prices for tournaments or playoff games may vary from the ticket prices shown below. Family passes cannot be used for admission to any tournaments or playoff games. All levels of teams (V, JV, C etc.) are charged admission at ticketed events.
