Family Compact & Important Family Information

At Mill Plain Elementary, we believe that a strong partnership between families, students, and school staff helps every child succeed.

Please review the following documents for important information about our shared commitments, your rights as a family, and the additional academic support available to students.

1. Family Compact

The Family Compact outlines how students, families, and school staff will work together to create a positive and successful school year.

2. Family Right to Know Letter

Under the Every Student Succeeds Act (ESSA), families have the right to request information about the professional qualifications of their child's teacher(s) and instructional paraprofessional(s).

3. Informed Notice of Services

This notice explains how Evergreen Public Schools uses federal Title I, Part A, and state Learning Assistance Program (LAP) funds to provide additional academic support in reading, math, or both. These services help address students' individual learning needs throughout the school year.

Complete the Family Compact Signature Form

After reviewing the Family Compact and the informational notices, families are asked to complete the Google Form below. The form is used to collect electronic signatures in place of physical signatures on the Family Compact.
