This section details instructions for externships at
Palomar Health ONLY!
All PH & PHMG externships are completed in ONE 40 hr. work week, Monday-Friday 7:30am-4pm. They do not accommodate personal schedules. If you want the spot, you have to adhere to the schedule and be ready to show up every day when scheduled. If you miss your externship dates, you will not be eligible for another externship with EAS. Plan accordingly!
These placements are the most coveted and very popular because students receive the most competencies practiced in this externship, it most likely leads to a job offer, and they provide very competitive pay and benefits. That being said, Palomar also requires the most comprehensive onboarding requirements because they want highly motivated, intelligent candidates who can follow directions and complete tasks in a timely and professional manner. It is the STUDENT'S responsibility to navigate this process and figure out a way to complete each of the instructions.
In addition to these instructions, you must complete and adhere to ALL the EAS externship protocols detailed throughout the website.
The compliance and onboarding process typically takes 1-2 months depending on the student's ability to complete these steps. Please plan accordingly.
Phlebotomy Externship Clearance through Complio and EAS
STEP 2.1: Use the EAS Phlebotomy Clearance Guide to complete all of the necessary Complio and EAS requirements for requesting a phlebotomy externship with Palomar Health.
Please understand you will be solely responsible for the fees associated with the on-boarding/clearance process.
The clearance guide will provide all of the specific information as to how to satisfy each component of the clearance process.
Two out of three students have their profiles and externship placement requests rejected because they do not follow directions. Be sure to READ every word of the clearance guide before starting your profile and then continue to check off every component of the guide as you go to avoid missing essential deadlines or details required.
Completing this clearance packet does not guarantee placement. You still have to follow through with all of the EAS requirements listed on this website and adhere to the protocols and practices we outline beyond this initial phase of requesting an externship.

Palomar Student Health Clearance Packet (ONLY these documents are accepted, no other attachments or documents will be compliant)
NEW UPDATE: Palomar notified us there has been a change to the MMR category in Complio. You must select a manufacturer name for both the 2 or 3 dose series. Please see the following notice,"If your students have 3 doses transcribed by the provider and the provider selected “3-Dose series”, select any manufacturer other than Heplisav-B and add the form to HBV – 3 Dose Series – Dose 1, 2, and 3. If the provider selected “2-dose series Heplisav-B formulation only” then for HBV – 2 Dose Series – Dose 1 and Dose 2 indicate Heplisav-B. We were having students who submit their medical records try to become compliant with only 2 doses of a non-Heplisav-B formulation so we’re just trying to prevent that." This is a temporary change until the Palomar health clearance packet is updated to match the Complio category requirements.
STEP 2.2: Once all Complio verification is received you will need to request your Complio package be cleared by EAS.
Access the Complio Clearance & Start Date Request Form to request your Complio package to be reviewed for EAS clearance and to request an externship start date.
The date you request must be no EARLIER than 14 business days from when you request Complio Clearance.

You should receive an email with either confirmation of compliance or rejection with steps to clear your profile in 3-5 business days. Your start date will also be confirmed or rejected depending on your clearance status.
STEP 2.3: Once the request is approved, a Clinical Rotation will be made on your behalf and you will receive notification to complete the "Clinical Rotation" tab in your Complio account.
Go to Step 3 and follow the instructions to fulfill your Clinical Rotation requirements.
Please refer to the EAS Phlebotomy Clearance Guide for specific instructions.
Once students have been preliminarily approved by Complio and the EAS Externship, a "Palomar Health Allied Health Rotation Package" will be requested on your behalf and a new "Tab" labeled "Clinical Rotation" will appear at the top of your Complio Dashboard.
You must complete and submit any requirements in Complio for your Clinical Rotation at least 14 BUSINESS DAYS before your start date.
Your final clearance step will be to review the specific instructions detailed in the clearance guide as to how to show up for your first day.
Please go to STEP 4 for the instructions and documents you will need to bring with you on each day of your externship shift.
Once students secure an approved externship placement they will need to:
Fill out the Current Externship Form confirming your start date and location.
Access the EAS Phlebotomy Uniform Standards & Code of Conduct document to review what is expected of all externship candidates in regard to professional appearance and professional conduct.
Download and read thoroughly, all the required externship documents provided.
Make sure you are using the CORRECT documents for your specific externship.
Ask for clarification or any questions regarding requirements detailed in the externship documents provided.
Properly complete and attain all signatures each document requires in order to receive externship certification.
Print and/or fill out each document digitally or using a pen. All signatures required must be hand-signed.
All documents requiring a signature must be returned to the Externship Coordinator for verification and processing of certification.

The following sections provide detailed instructions for each of the externship documents you will use during your externship experience.
Students and supervisors are expected to:
Review the competencies provided. This document contains the skills and knowledge gained throughout your classroom theory and practice. They are not provided to serve as a "need to complete" list of action items when completing your externship hours.
Use the occupational competencies document as a guide for hands-on practice of the knowledge, skills, protocols, and procedures a working employee would be expected to perform or understand on the job.
Ask their supervisor to provide when appropriate, hands-on training or experience for each competency listed.
Develop a schedule and plan to discuss or practice each competency during their scheduled shifts over the course of the 40 hours of externship.
The Phlebotomy Training Plan is required for students to use to:
Document when a student experiences each competency and whoever provides the student their training.
Although a student might have one supervisor for their externship, different medical facility employees can provide students training to meet the expectations for the competencies listed in the training plan.
Log each competency experience with:
The trainer's full name
The date when the experience occurred
The supervisor's initials for each competency listed.
Sign and date the training plan document
Attain their supervisor's signature and date of signature.
The Phlebotomy Training Evaluation is required for the externship supervisor to use to:
Evaluate a student's externship performance.
Preview each of the evaluation criteria with the student before they begin their externship.
Keep an open dialogue regarding the evaluation expectations.
Students should:
Ask your supervisor to communicate any concerns they have regarding your performance as they occur. This will give you the opportunity to adjust or improve your performance before your externship ends.
Ask your supervisor throughout your externship if you can do anything to improve your performance.
Be open to constructive criticism.
If you have any concerns regarding your experience during your externship, please contact your Externship Coordinator immediately. This will give us the opportunity to assist you with any issues that may arise.
Students are required to:
Attain their supervisor's signature and date of signature.
The Phlebotomy Training Time Sheet is required for the student to:
Log the scheduled shifts throughout the 40 hours of the externship term.
Students should:
Discuss their weekly availability as well as the availability of hours they can schedule monthly with their supervisor prior to beginning their externship term.
Although not required, students should try to work out a schedule with their facility that allows them to complete their externship in no more than 4 weeks. This would allow students a minimum of 10 hours a week and provide a steady schedule to meet the goals of the 40-hour externship.
When a schedule is agreed upon, students are required to:
Complete the externship timesheet weekly with the required signatures for their supervisor.
Make sure to use the "Time Sheet Instructions" document to correctly fill in the hours worked.
You may make as many copies of this document as necessary to log all of the hours and weeks you complete throughout your externship.
Attain their supervisor's signature and date of signature.
You will need to have the externship site lab supervisor fill out the "Clinical Training Site" box. The most important information is the CLIA number. You will not be able to request a phlebotomy license without a valid CLIA number filled out on this form.
The Phlebotomy Practical Training Attestation is required for the student to have:
Verified and signed by a qualified medical professional.
This document certifies the student has completed the Certified Phlebotomy Technician 1 (CPT1) requirements of 50 venipunctures.
Students are also required to complete 10 capillary punctures. The appropriate "Skin Punctures" and Arterial Punctures" boxes that are filled in at the bottom of the form apply to your practical experience in class.
Students should:
Make sure to discuss when they will have opportunities to practice capillary punctures and venipunctures and who will be able to witness punctures. Students need to make sure they do not wait until the end of their externship to try and complete the requirements of this certificate.
Students are required to:
Attain their supervisor's "wet" signature and date of signature. Digital signatures are not accepted.
The date of "Signature by the Clinical Supervisor" MUST match the end date of clinical training in the "Phlebotomy Training Student Information" box above.
Blood draw records and the California Practical Training Assignment and Attestation Form must be signed by a certified doctor, physician assistant, nurse, or phlebotomist (MD, DO, PA, RN, RCP, NP, CLB, CLS, CPT, NMT, or MLT).
Do not forget to include the supervisor's clinical license number!
The Venipuncture and Capillary log forms must be used to document your supervised and successful draws.
How to fill out these logs:
Document all SUCCESSFUL punctures you do during your clinical training.
You must log a minimum of 50 Venipunctures and a minimum of 10 capillary punctures.
Make sure to use the correct log for each different type of puncture.
(Venipuncture only) Please indicate whether the draw was using a vacutainer or syringe by indicating a “V” or “S”.
(Venipuncture only) If the draw was performed using a butterfly please indicate with a check mark “✔”.
Please indicate what type of patient the draw was performed on; A=Adult, E=Elderly, C=Child.
If you performed the draw at a different site other than the assigned externship location, please indicate the name of the site in the column provided.
Please have your supervisor determine if the draw was successful by indicating a “Y” for Yes or “N” for No.
Please have a qualified and certified clinical supervisor witness and attest to the draw by signing their initials in the column provided.
You are welcome to number and log additional successful draws in the additional rows provided on each log. The more recorded draws the better!
Do not lose your logs!! It would be wise to take a picture or scan the logs once you reach the minimum draws needed just in case you misplace the paper logs.
Students are responsible for completing all of the steps outlined in the externship process.
Once all hours are completed, competencies are met, all documents are filled out appropriately, and all signatures required attained; students will need to:

OR, students can drop them off at our Escondido Crest campus in a sealed manilla envelope with their full name and program name clearly labeled on the front.

Congratulations on completing the phlebotomy course externship!
In order to practice phlebotomy in the state of California, you must have your California State License.
Please follow the steps below carefully.
You must complete Steps 1 through 4 PRIOR to applying to the State of California for a Phlebotomy License.
Step 1: Successfully pass the phlebotomy class and all 40 hours of attendance.
Step 2: Successfully pass the Phlebotomy Certification Examination and receive your national license certification.

*This document is to assist you with the process. We are not responsible for any changes in the procedures, protocols, or steps to completing the NHA CPT I exam that NHA may make at any given time. It is your responsibility to register, schedule, pay, and complete the exam according to NHA guidelines.*
Step 3: Complete a phlebotomy externship of a minimum of 40 hours and receive an externship certificate from the Escondido Adult School.
Step 4: After steps 1-3 are completed and verified you will receive a certificate of completion from the Phlebotomy Training Academy.
Step 5: Apply to the State of California for the Phlebotomy License.
Create an account at the California Dept. of Health to "Request access to the Laboratory Field Services application". Save your login credentials.
Once you have created an account visit the CDPH - CPTI Certificate webpage.
Here, you will find information regarding your requirements. READ the requirements prior to applying!
At the bottom of the page, click on the "Apply Online" button under the "How to Apply" section. You will be taken to another website and asked to use your account credentials to sign in.
Login to your account and follow the directions carefully.
You will need:
Your National Phlebotomy Certificate from passing the NHA exam.
Your completed "California Phlebotomy Practical Training Assignment and Attestation". (From your externship packet. Please add the year to the signature date)
Your Certificate of Completion from Phlebotomy Training Academy.
Official High School Transcripts - be sure to follow the instructions *exactly* as they are written on the CDPH - CPTI Certificate webpage or your application will be "abandoned".
You will then pay the application fee and wait for processing.
If ALL of the documents and instructions are not completed as stipulated, the CDPH will "abandon" your application and you will have to re-apply AND pay the application fee again.
Our office cannot (per CDPH policy) and does not assist with the State of California Licensing process.
