To complete the Athletic Clearance Process, all forms must be signed on athleticclearance.com. The sports physical must be dropped off at the front office.
Please note: An online account must be created by the athlete on https://www.athleticclearance.com/. If this account is not created and completed at the time of form drop off, the scholar will not be cleared.
Insurance Information
Under state law, school districts are required to ensure that all members of school athletic teams have accidental injury insurance that covers medical and hospital expenses. This insurance requirement can be met by the school district offering insurance or other health benefits that cover medical and hospital expenses. Some pupils may qualify to enroll in no-cost or low-cost local, state or federally insured program. Information about these programs which include other comparable no-cost or low-cost local, state or federally sponsored health insurance programs, may be obtained by calling Healthy Families and Medical Programs Information Line at 1-800-880-5305, Medi-Cal at 1-866-262-9881 or Pacific Educators at 1-800-722-3365.
The Escondido Union High School District does not provide medical, accident or dental insurance for pupils injured on school premises or through school activities. In accordance with Education Code Section 49472, the District does make available a low-cost medical/dental accident insurance program. The purpose of these plans is to provide assistance at a minimum cost to meet some of the expenses for accidental injury.
