Step 4: Prepare Residency Documents for submission (as indicated from Step 2) AND
the following important documents
Online Registration Application (Print out the Online Registration Summary for submission)
Official Birth Certificate (government issued only, not from hospital); foreign certificates MUST be translated
Parent/Guardian’s Current California Driver’s License/Identification Card
Immunization Record/this must include the record of Tuberculosis (TB) Test
Santa Clara County Public Health Dept. TB Risk Assessment form. ONLY for students enrolling from outside of Santa Clara County. Form available at the Registrar’s Office and on our website:https://evhs.schoolloop.com
Transcript (unofficial or official) from previous school
Withdrawal paperwork from previous school
Current IEP or 504 must be submitted at the time of registration (If Applicable)
Once we have received ALL your residency verification documents, other important paperwork AND have a complete registration form, we will send you a confirmation email that you have completed ALL the registration steps above. At this point no further action is needed from you. The next time you will hear from us will be in late July/early August to share information about the start of school year festivities AND when to pick up your student schedule.
