In order to obtain working papers you need:
*Section 1 of the application filled out by the student and signed by your parent/guardian
*A physical performed within the last year. (A copy of the physical can be provided OR you can have your health care provider or the school nurse sign off on the Physical Fitness Certification.)
Bring both items into the Counseling Office Secretaries

Paper copies are available in the Counseling Office.

This certification must be filled out by a healthcare professional or by the school nurse (if the school nurse is in possession of a physical performed within the last year)
OR you can bring in a copy of your physical performed within the last year.
