Indicator 12 measures the percentage of children referred by IDEA Part C (Early Intervention) who are found eligible for Part B (Preschool Special Education Services) and who have IEPs developed and implemented by their third birthdays.
The Office of Early Learning and School Readiness reviews prior school year data for all districts reporting at least one child transitioning from Part C in EMIS. To determine compliance, the Office of Early Learning and School Readiness compares the child’s date of birth to the initial individualized education program effective start date.
New this year, all districts flagged for an indicator must upload Policies, Procedures and Practice documentation that corresponds with the indicator in question. Additionally, those flagged for indicator 12 must upload the most current Interagency Agreement, all by February 7, 2025. These documents will be reviewed and feedback given. If changes are to be made, submit the corrections by September 19, 2025 along with the systemic improvement evidence.
If the district is proposing it has been flagged for the indicator due to incorrectly reported data, the district will complete the Indicator 12 Data Reporting Error Verification Form. Additionally, the district will list each State Student Identification (SSID) number listed on the Records for Review Excel spreadsheet for Indicator 12 on the Indicator 12 Student Records Submission Form (Due: 2-7-25).
Districts flagged with noncompliance due to underreporting will complete the Indicator 12 Transition Data Tracking Form for all children transitioning from Part C to the district between the first of August and the first of November of that school year, including children who were not suspected of having disabilities, children determined not eligible for Part B (preschool special education), as well as children determined eligible for Part B.
Districts with noncompliant timelines with complete the Indicator Analysis and Improvement Plan for Indicator 12 with the support of the SST3 (Due: 4-4-25).
The final step to the process will include completeing the actions in the approved Improvement Plan and submitting that evidence as described in the Indicator 12 Guidance Document by September 19, 2025.
