Our Applications are both for Families and a potential provider. Below is the application for each.
For a parent/ Guardian Application the following documents will need to be attached when submitting.
Child's CIB or Enrollment Form, Immunization Record for all Children, Employment Pay Stubs for 1 Month or School Schedule/ Award Letter.
For Provider Application the following documents will need to be attached when submitting:
W-9 Form, Date of Birth Document. ID/ Drivers License and a Negative Tuberculosis Test Record.
