College Department

All college application must be processed onsite. Please visit the LCUP DTC Office, LCUP Main Campus, Gate 1 to submit all the necessary documents:

• 1pc 2x2 Picture (White Background)

• Original PSA Birth Certificate

• Original Card (Form 138 or SF 9)

• Entrance Examination (P400.00)

• Original Good Moral Certificate

• Transfer Credentials (For College Transferees)

• Original PSA Marriage Certificate (If Married)

• ALS / PEPT Certificate (If Applicable)

College students who wishes to apply to the LCUP FAP (Partial Scholarship), please submit the following requirements:

• LCUP FAP Online Application Form

• 1pc latest 2x2 Picture

• Photocopy of Grade 12 First Semester Card (for Evaluation) and Grade 12 Second Semester Card (for Approval)

• At least an Average or passing mark in the LCUP Entrance Examination (P400.00)

• Photocopy of PSA Birth Certificate

• Photocopy of Certificate of Good Moral Character from the previous school

• Certificate of Residency from the Barangay

• External and Internal photos of the residence.

• Most Recent Utility Bill with complete address of the applicant

• Notarized Affidavit of Source of Income with complete address
