Required Forms

Physical Exams are required for pre-k, kindergarten, and 6th grade

  • Espanol: El Examen de Salud 

Dental Exams are required for kindergarten, 2nd grade, and 6th grade

  • Espanol: Examen Dental 

Vision Exams are required for kindergarten

Transfer Students

• Physical Examination including all required immunizations must be completed by appropriate medical personnel.  Out of the state/country health records must be on an Illinois Health form or comparable form dated within one  year of the first day of school. This must be received by the school within 30 days of enrollment. 

• Vision Examination – Required for K, or any student entering an Illinois school for the first time at any grade. This  exam is to be submitted within 30 days of enrollment.

Sports physicals are required prior to playing any sport (including tryouts) at Elwood School. 

• • Illinois Sports Physical Form

  • When a student gets a school physical in 6th grade, there is an option for the doctor to check a box that says okay to play interscholastic sports- this covers 6th grade students for their sports physical.
