If your student has any of the following health conditions, please notify Nurse Michelle via phone  or in person prior to the start of the school year:

• Asthma

  • Asthma Action Plan

• Seizures

  • Seizure Action Plan

• Diabetes

  • Diabetes Medical Management Plan

• Allergies

  • Food Allergy and Epi-pen paperwork

• Cardiac Conditions

If your student will need to take medication at school please have the doctor fill out our school's medication form:
