Medication administered at School

Any student needing to take medication while at school will need a medication authorization form filled out and sent to the school nurse. 

• If it is an over-the-counter medication such as Tylenol, Motrin, Benadryl, or Zyrtec then a parent or guardian will need to fill out the top portion of the form, sign it and return it to the school nurse. *ANY medication that is to be given daily for 72 weeks will require a Doctor's order.

• If it is a prescription medication, then the top and bottom portions will need to be filled out. The doctor that prescribed the medication must fill in the bottom portion of this form, sign it and return it to the school nurse.
