Applicant Information - Complete a Google Form with your contact information
Criteria #1: On Track toward Graduation - Answer a question about your progress toward graduation.
Criteria #2: Knowledge of U.S. History and American Government - Indicate if you have passed or are expected to pass these classes.
Criteria #3 A: Civic Action Project/Civic Internship - Complete a Google Form and answer two questions about your Civic Action Project or Civic Internship.
Criteria #3 B: Civic Project/Civic Internship Evidence - Upload or provide links with evidence that documents your Civic Action Project or Civic Internship.
Criteria #4 Self-Reflection - Complete a Google Form and answer two self-reflection questions in the form about your Project or Internship.
Criteria #5 Civic Mindedness - Complete a Google Form with contact information for your adult reference(s) and optionally list activities that demonstrate civic mindedness.
