Complete pg. 1 of the permit, as well as all three sections on pg. 2 (minor’s information, parent signature, and employer information). These sections must be completed before submitting your work permit.
Email option: Scan and email the completed work permit to syurkovic@eduhsd.k12.ca.us
OR
Turn in your completed work permit to the College & Career Center in Room A-210.
The permit will be processed and returned to you. Please allow 24 hrs for processing.

Students and parents/guardians requesting a work permit must sign pg. 1 of the "Request for Work Permit" which also acknowledges that if the student falls below a 2.0 GPA or receives an 'F' while employed, their permit will become void and they will no longer be allowed to work.
