In particular, the school needs to know if your child has any severe allergic reactions (anaphylaxis), asthma, diabetes, cystic fibrosis, epilepsy, haemophilia, severe anxiety, or any other diagnosed conditions.

Please ensure any relevant up-to-date action plans and medication is provided to the school by Wednesday 28th January.
