The paperwork you need

(You can get all these documents in my office - 1G.02 if you prefer to take it to your host employer in person)

Below is the Student  Placement Record that must be filled in and returned to me BY THE END OF TERM 3.

Section 1 is done by the student. Write your full name and the host's name at the top of every page.

Section 3 is by the employer. 

Section 4 is done by the family. 

Once all those have been completed, it return to me to be checked and approved.

You can either print it out and bring it to me, or you can email it to adam.westwood6@det.nsw.edu.au.

The Travel Form is necessary if you will be in a vehicle with the Employer

It is possible for the student to stay overnight for work experience, but there's some extra paperwork that needs to be completed in order for that to happen. Have your family call me and I will help them through the process.
