STDF 2024 - School participation form - to nominate the number of items you wish to participate in the festival.
Due by Friday 10 May 2024 (Term 2 Week 2)
STDF 2024 - Item information form - a separate copy of this form is required for each item from your school.
Due by Friday 24 May 2024 (Term 2 Week 4)
music file/s - upload your final music file/s to the STDF 2024 Teacher drive - Music files folder. Please submit a separate music file for each item. Label files in the folder with school and item name(for example: Dawson PS - Let's dance.mp3
lighting design plan/s - upload your completed lighting design plan/s into the STDF 2024 teacher drive - lighting plan folder. Please submit a separate lighting plan for each item. Label files in the folder with school and item name(for example: Dawson PS - Let's Dance - lighting design.docx
student and staff roll - upload your completed student and staff roll/s into the STDF 2024 teacher drive - student and staff roll folder. Please submit a separate roll for each item. Label files in the folder with school and item name (for example: Dawson PS - Let's Dance - student and staff roll.docx. This roll will be used for program names so please ensure accuracy when typing names into the document.
STDF 2024 - Student publicity consent form - to be collected and held by the coordinating teacher at the school. A Word document version of this form is available in the STDF 2024 - Teacher drive folder if you wish to amend this form to add to your school excursion permission note.
STDF 2024 - Performance day confirmation form - Please complete this form to confirm staffing, student permission to publish and principal endorsement.
The online STDF 2024 - Item information form should take no more than 10 minutes to complete. Please note, that a separate form must be completed for each item from your school that you are submitting.
Once you have started your application, you will not be able to save your progress and exit it. It will not time out, however, if you close the window, it will exit.
To ensure you are able to complete your application properly and all in one go, please make sure you have the information contained in this checklist available before completing your application.

Declaration:
I have read the STDF teacher information and have worked to create dance work/s suitable to the age and ability of my students.
I warrant that the dance work/s created for the festival are original works.
I give permission for this work/s to be videoed, reproduced in digital files and photographed by the NSW Department of Education for educational, archival and promotional purposes only.
I have obtained any necessary copyright permission for music, motion picture soundtracks, sound effects and dramatic dialogue utilised in my work.
My principal approves and endorses the above-mentioned student dance group/s and teacher/s participation in the STDF.
My principal has viewed our performance in full and can confirm that the song lyrics, costumes and dance movements are acceptable to the school community and will support the values of public education.

Please note:
Teachers are responsible for supplying their own first aid kit and collecting medical information for students, in line with normal school excursion policies.
