NJ Minimum Immunization Requirements for School Attendance

Note to Graduating 5th Grade Parents/Guardians

Students entering Grade 6 and 11 years of age-Notification of Required Immunizations for 6th Grade Students

New Jersey State Law requires ALL students who are entering 6th grade AND are 11 years old must receive an immunization of a Tdap booster and a  Meningococcal vaccination.
