ezSCRN Information

Beginning Monday, September 21, 2020, you will need to complete an attestation to your children’s health on a screening app before they arrive at school each day. One screening must be completed for each child, each day. The app we are using is called ezSCRN.

You should have already received an email directly from ezSCRN with directions. (Please also check your SPAM folder.) Please read the email from ezSCRN carefully and create your account.

• If you have an iPhone, you can use the iPhone app.

• If you are not using an iPhone, please use another device and login at app.ezscrn.com

• If you have trouble getting ezSCRN to work, please fill out this form. Technical support will be provided through this form only- please do not email school staff.

• If you cannot log on to the ezSCRN app or website on a day your child is attending school, please use this back-up form. Send the completed form to school with your child.
