Parent Input and Request for Assistance : 

A student can be brought to the SST by a campus staff member, or a parent/guardian.  Parents/guardians may fill out the Request for Assistance form below.  The SST will review the request and will gather data to determine if the student requires any supports.

Student Self Assessment:

Students, please complete the following Self Assessment Form if your parent/guardian has completed a Request for Assistance Form.

Requesting a Review of an Active Section 504 Plan:

Parents/Guardians and Students may request a review of an existing Section 504 plan.  Plans must be active for at least an academic year before reviews are scheduled unless there are extenuating circumstances (new diagnosis or updated evaluation, the student is in crisis, etc).

Requesting Standardized (College Entrance) Testing Accommodations:

Some students may qualify for accommodations on college entrance exams.  Decisions regarding accommodations on ACT and College Board (SAT, PSAT, and AP exams) are made by those entities and not by Westlake High School. The link below will take you to the WHS College Accommodations Information Page.
