Please fill this out if you would like to be cleared as a driver for our many competitions, festivals, and more!

Necessary documentation to attach to form:

• Complete Volunteer Screening Form w/ copy of Driver's License

• Copy of insurance declarations page

• Proof of good driving record (e.g. good driver discount, document showing points from DMV)

• Proof of vaccination

Please fill this out if you will drive/pickup your own student to/from a band or colorguard event outside of Fallon Middle School.
