Since the Sells Middle School Athletics Coach Staff has limited ability to meet with 7th and 8th Grade students to discuss the upcoming season, this site has been created to sign your child up for Sells Middle School Winter Sports.
REGISTRATION PROCESS
#1- Complete Google Form. Click on the sport above that you are interested in participating in the Winter, and complete the form embedded in the page. This information will be received by the Head Coach and your child will be "signed up." Please sign up no later than 10/26/2020. Contact the coach directly if you need to sign-up past this deadline.
If your child plans on trying out for a cut sport, you can sign your child up for that sport AND a non-cut sport (as a back-up plan).
#2- Final Forms. Create a Final Forms account at https://dublin-oh.finalforms.com. Both the parent AND student-athlete must create a Final Forms account AND sign all necessary forms for the Fall. The account needs to be created and all forms need to be signed by 10/26/2020. Make sure you specify the sports your child plans on trying out for/playing on Final Forms. This will ensure you are on the coaches' e-mail group. Coaches will contact all those who sign up by mid-October to inform you of any details regarding practice/try-outs.
#3- OHSAA Physical. Schedule a Physical Exam with your family physician. Have him/her complete the required OHSAA Physical Form (this can be found below). These are valid for (1) year. A valid form needs to be submitted to Athletic Director Jay Schwanke or Head Trainer Pam Stotts by 10/26/2020.
#4- COVID Waiver. Please make sure you complete and sign the Dublin City Schools COVID Waiver Form for athletics. The hard copy needs to be printed, completed, signed, and given to the Head Coach on the first day of open gym or open mats.
COVID GUIDELINES
The Dublin City Schools COVID Waiver is a requirement for your child to participate.
If your child is experiencing COVID symptoms (fever, cough, runny nose, shortness of breath, etc), please KEEP YOUR CHILD HOME and contact the coach.
All coaches and athletes must wear masks in the facility at all times (unless they are active in the field of play).
Coaches will have a plan in place to socially distance all participants.
No water will be provided. Your child must bring his or her OWN water (at least 32 ounces). No exceptions.
Please have your child bring his or her own hand sanitizer for during practice.
