Step 1: Send a Request letter
Staff seeking assistance to submit a Letter Request addressed to the Regional Director thru the Human Resource Welfare Section stating the need for assistance, type of assistance and for whom is the assistance intended for. Can be submitted personally or sent thru email or FB Messenger.
Step 2: Submission of Documentary Requirements
Submit the documentary requirements based on the type of assistance requested to the Human Resource Welfare Section (HRWS)
Step 3: Assessment and Facilitation
Assess the current need of the employee/dependent.
Check the submitted documentary requirements.
Accomplish the Employee Assistance Sheet (EAS).
Preparation of endorsement letter to Crisis Intervention Section (CIS).
Step 4: Accomplish the Client Satisfaction Survey Form
Upon receipt of the endorsement letter, the requesting staff shall accomplish the Client Satisfaction Survey Form.
Step 5: Submission of Documentary Requirements to Crisis Intervention Section (CIS).
Assist the requesting staff in the submission of the documentary requirements to the assigned Social Worker
