If you are covered by a different insurance company, your health insurance plan likely provides some level of out-of-network reimbursement. I am happy to provide the required documentation you'll need to submit to get partially reimbursed by your insurance company.
Fees for Psychological Diagnostic Evaluations

Psychological diagnostic evaluation without medical services: $225

Pre-Employment and Fitness For Duty Evaluations
On a case by case basis, I do psychological Fitness for Duty and Pre-Employment Evaluations. Health insurance does not pay for them. These evaluations take 1 - 3 appointments to complete. They require the participant to complete in-depth standardized assessments on the computer and a 1 - 2 hour psychological interview. Please contact me to discuss my fee structure for these evaluations.
Good Faith Estimate
You are entitled to receive a “Good Faith Estimate” of what the charges could be for psychological services provided to you. While it is not possible for a provider to know, in advance, how many psychotherapy or medication management sessions may be necessary or appropriate for a given person, this statement provides an estimate of the cost of services provided. Your total cost of services will depend upon the number of psychotherapy sessions you attend, your individual circumstances, and the type and amount of services that are provided to you.
There may be additional items or services I may recommend as part of your care that must be scheduled or requested separately and are not reflected in this good faith estimate.
This estimate is not a contract and does not obligate you to obtain any services from the provider(s) listed, nor does it include any services rendered to you that are not identified here.
You have the right to initiate a dispute resolution process if the actual amount charged to you substantially exceeds the estimated charges stated in your Good Faith Estimate (which means $400 or more beyond the estimated charges).

The initiation of the patient-provider dispute resolution process will not adversely affect the quality of the services furnished to you.
Psychological Diagnostic Evaluation and Medication Management Services:
The fee for an initial psychological diagnostic evaluation with medical services is $275. The fee for follow-up 25-minute medication management visits (in-person or via telehealth) is $175. Most clients will require one medication management visit per month, but the frequency of visits that are appropriate in your case may be more or less than once per month, depending upon your needs. Based upon a fee of $275 for the first visit and $175 per follow-up visit, if you attend one medication management visit per month, your estimated charge would be $2,200 for the first year of visits.
Psychological Diagnostic Evaluation and Psychotherapy:
The fee for an initial psychological diagnostic evaluation without medical services is $275. The fee for follow-up psychotherapy visits (in-person or via telehealth) is $225 for sessions lasting from 53 - 60 minutes, $175 for sessions lasting between 38 - 52 minutes, and $125 for sessions lasting between 16 - 37 minutes. Most clients will attend one psychotherapy visit per week, but the frequency of psychotherapy visits that are appropriate in your case may be more or less than once per week, depending upon your needs. Based upon a fee of $275 for the first visit and $175 per follow-up visit, if you attend one psychotherapy visit per week, your estimated charge would be $800 for the first four visits provided over the course of one month; $1,500 for the first eight visits over two months; and $2,200 for the first 12 visits over three months. If you attend therapy for a longer period, your total estimated charges will increase according to the number of visits and length of treatment.
This Good Faith Estimate is not intended to serve as a recommendation for treatment or a prediction that you may need to attend a specified number of psychotherapy visits. The number of visits that are appropriate in your case, and the estimated cost for those services, depends on your needs and what you agree to in consultation with your therapist. You are entitled to disagree with any recommendations made to you concerning your treatment and you may discontinue treatment at any time.
