Referrals

Referrals are required - Ask your Family Physician (or walk-in clinic) to send a Referral to Dr. Athaide.

Self-Referrals can be made for pregnant patients who have not been seen by any other provider (Family Physician/Walk-in Clinic/Midwife/other Ob) during this pregnancy. If you have already been seen by someone else, have them submit a referral (required).
