Each year, respiratory viruses are responsible for millions of illnesses, thousands of hospitalizations, and thousands of deaths in the United States. Staying up to date with your recommended immunizations can reduce the risk of severe disease, hospitalization, and death. It is especially important for those at higher risk of severe illness from respiratory viruses to stay up to date on recommended immunizations. (CDC.gov)
MCCCD will be offering Flu Vaccines during the In-Person Expos.
Afluria Quadrivalent – the standard flu shot for adults under age 65.
Fluad Quadrivalent (High-Dose) – recommended for individuals 65 years and older for added protection.
Flu Shots Available on a Walk-In Basis
Flu shots will be administered on a walk-in basis — no appointment necessary.
To access or download your flu shot records, you will need to create an account. Please follow the instructions below to set up your account.
Biometric screenings will be by appointment only. See registration directions below.
Please note: If expos dates and times do not work for you, off-site screening using Quest or LabCorp vouchers will also be available through the registration link below from October 31, 2025 - November 21, 2025.
Flu Shots and Biometric Screenings will be brought to you by Health and Wellness Professionals. Scan the QRCODE below, or sign in to the Health & Wellness Professionals MCCCD website to begin your registration.
When registering your account.
Use the ID number as listed on your Aetna Insurance card (this is your employee ID with a zero "0" in front of it).
Use the First and Last names as listed on your Aetna Insurance card.
Once you submit your registration information, a confirmation email will be sent to you to complete your registration.
During the registration process, you will be asked if you want a flu shot. Your flu shot record will be uploaded to your account, allowing you to download it if needed.
From the link provided, you will be taken to your personal log in page where you will be able to create a password.
You can also go into your profile and enable sms messaging and two-factor authentication.
If you selected "No" for Flu Shots when registering, and change your mind, this can also be updated within your profile page.
A biometric screening helps you identify and monitor concerning health conditions, and promotes engagement in preventative health so you can make educated decisions about your healthcare and behaviors. It includes assessments of Cholesterol, Blood Sugar, Blood Pressure, Height, Weight, BMI, and Waist Circumference. Disclaimer: Screenings are preventive, not diagnostic. HbA1c testing will be performed only if eligibility criteria are met. (An HbA1c test is a blood test that measures your average blood glucose (sugar) level over the past two to three months. This test will only be performed if the clinical eligibility criteria are met during the biometric screening.)
If you are enrolled in an MCCCD Medical plan and engaging in the Personify Health Wellness Platform, you can participate in one of the upcoming Open Enrollment In-Person Expos and earn points toward your Wellness Incentive Credits:
Earn “Participation Rewards” for attending an In-Person Expo/Event: 250 points.
Complete 3 preventative care activities to earn 300 points. One of those activities can include getting your Flu Shot.
After completing your flu shot, log in to your Personify Health account.
From the top dropdown menu items, select Health > My Care Checklist
Under the “Preventive Care” section, locate the “Flu Vaccine” option.
If this is not showing, select the Edit button, and enable the Flu Vaccine option.
Select “Add Date,” enter the appointment date, and then save.
Your completed appointments will be updated with a new date and marked with a green check mark.
Complete a biometric screening: 1000 points.
Complete a biometric screening (either onsite or offsite using a Quest or Labcorp voucher during the open enrollment period, October 31, 2025 - November 21, 2025). Appointments are required, and campus appointments can be made in advance. Offsite vouchers will be available to download starting Friday, October 31, 2025.
There is no form or proof that you need to submit to earn these points. Once you have completed the biometric screening, it will be coded accordingly and identified by Banner|Aetna. Banner|Aetna will provide a monthly listing of individuals who have met this requirement. No additional information regarding the visit will be provided to the MCCCD Benefits Team or Personify Health.
Please NOTE: A biometric screening helps you identify and monitor concerning health conditions, promoting engagement in preventive health so you can make informed decisions about your healthcare and behaviors. It includes assessments of Cholesterol, Blood Sugar, Blood Pressure, Height, Weight, BMI, and Waist Circumference. A biometric screening should not replace your yearly physical with your Primary Care Physician (PCP).
Earn “Redeem A Voucher” reward by Confirming Your Benefit Elections: 200 points.
This Voucher will be available to redeem between October 31 - November 30, 2025. The voucher code will be provided at the end of your ADP confirmation statement.
Redeem your eligible points on your Personify Health account by completing the following steps:
Log in to your account on the Personify Health member website.
Step 1: To ask a question, utilize the Ask Benefits Help Desk ticket system.
Step 2: If you need to submit confidential information, please send an email to rx@domail.maricopa.edu and include your Help Desk Ticket number.
The best practice when sending personal information via email is password-protecting your document(s). Therefore, we are including instructions for both a PC and MAC on zipping and password-protecting your document(s) below.
