• Processing days will be 10 working days, excluding Saturdays, Sundays and Holidays; no express requests are processed at this time. 

• For current student, please use your DLSU email address when accessing the Google Forms.

• Only registered parents or authorized guardians listed in our database are permitted to request documents on behalf of the student.

• Google forms for College applications will be opened for a specific period ONLY, in time with the College application period of each University.
