STEP 1: SUBMISSION OF ACCOMPLISHED AND SIGNED TRASTS FORMS

• Completed and signed Information Sheet
• Notarized Provisional Enrollment Form
• Completed and signed Acknowledgement Letter (for Filipino Applicants)
• Completed and signed Acknowledgement Letter (for Foreign Applicants)
• Completed and signed Applicants Personal History Statement
• Completed and signed Special Needs Assessment Form
• Completed and signed Declaration of Citizenship

STEP 2: CHOOSING MEDICAL EXAMINATION SCHEDULE & UNIFORM MEASUREMENT
As preparation for limited face-to-face classes for SY 2025-2026, all incoming freshmen must undergo a Medical Examination at De La Salle University Medical Center (DLSUMC) as a part of the Admission Requirements. On the same day, all incoming applicants may have their Uniform Measurement.

STEP 3: EARLY ENROLLMENT
Once you've received a confirmation email, please log in to your applicant's portal for the Online Enrollment Procedures.
After receiving your Registration Form, please like and follow our official Facebook page (DLSHSI Admission and Scholarships) for further announcements, such as the date of Regular Enrollment Start of Class, Senior High and Institutional Orientation, Academic Calendar updates, etc.
Note:
Once we release the final tuition fee, you can pay the remaining balance for you to be considered full payment (no installment fee) until the last day of our regular enrollment date.
